New Patient Forms & Information

If you are a new patient seeing Dr. Fry for primary care at the Stone Medical Building, 2800 N. Sheridan Rd.,  please click here to print a Primary Care Health Survey and fill it out prior to your first appointment.  Bring the completed health survey with you to your first appointment, along with items requested in links below that are specific to your location and needs.  If you have a copay, please bring that with you.  We accept cash and checks.  Thank you.  For pertinent information about our practice, please click here.
